Parent / guardian liability waiver
I, the undersigned parent or legal guardian, give permission for my child to participate in this program and acknowledge the risks associated with athletic activity, including (but not limited to) sprains, fractures, contact injuries, heat-related illness, and other injuries common to sports participation.
In consideration of my child's participation, I waive, release, and hold harmless the league, coaches, volunteers, and host facility from any and all claims arising from participation, except those caused by gross negligence or willful misconduct.
I authorize coaching staff and medical personnel to administer first aid and to seek emergency medical treatment as needed. I confirm that my child is physically capable of participating and has no condition (known to me) that would make participation unsafe.
I grant permission for the league to use photographs and video of my child taken during program activities for non-commercial promotional purposes.
By signing below, I confirm that I am the legal parent or guardian of the named participant and that I have read and agreed to this waiver on their behalf.
