Spring 5K Fun Run
A complete registration form for your 5K — adult and youth tiers, t-shirt size capped, liability waiver per runner, and a family-friendly cap on what one household pays.
Designed around how the event actually runs.
Running a 5K fundraiser means juggling registration, capacity, waivers, and pricing — all in a tool that probably wasn't built for it. This template handles all four. Drop in your event details, share the link, and let the form do the math.
- 2 registration tiers, each with its own capacity cap
- Per-participant info — names, contact, and event-specific fields
- Liability waiver signed per participant, with audit trail and PDF export
- Family pricing cap so households aren't double-charged for siblings
- Mobile-first form rendering — works on every phone your families have
- Auto-confirmation email to registrants on submit
- CSV export for the morning of the event
What your registrants will see.
This is the actual form, embedded — the same one your families will fill out on their phones.
Questions, answered.
Can I cap the race at a specific number of runners?
Yes. Each registration option (adult, youth, etc.) has its own capacity. When the cap is hit, that option closes automatically — no manual checking, no overselling.
How does the per-runner waiver work?
Every runner — adult or youth — signs the included race liability waiver as part of registration. Each signature is timestamped, stores IP and user agent, and is exportable as PDF. For minors, a parent or guardian signs on their behalf.
What is family pricing?
The family cap rule charges only the first 4 registrants in a single submission. A parent registering 5 family members pays for 4. You set the cap; it applies automatically at checkout.
Can I add t-shirt sizes with their own capacity?
Yes. Each runner picks a t-shirt size as part of registration. To cap each size individually (e.g. only 50 mediums), swap that question for separate registration options with their own caps after cloning the template.
Will the form work on phones?
Yes. The form is mobile-first — the parent registering at 9 PM on the school pickup line will have no trouble.
Does Attending take a percentage of my fundraiser?
No. Attending charges a flat rate, not a percentage. Every dollar your runners pay goes to your fundraiser.
